Job Description
Join UnitedHealthcare as a Field Based Care Coordinator, where you'll manage care for older adult members with diverse medical and behavioral health needs. Your role will be pivotal in developing personalized care plans and collaborating with community resources to improve health outcomes.
Key Responsibilities
Serve as primary care manager for high medical risks/needs members
Engage members face-to-face and/or telephonically for needs assessment
Develop and implement individualized, person-centered care plans
Partner and collaborate with internal care team and community resources
Provide referral and linkage as appropriate
Required Qualifications
Current, unrestricted independent licensure as a Registered Nurse in Indiana
Master’s Degree in Social Work
Bachelor’s Degree in Social Work, Psychology, Special Education, Counseling, or relevant degree with at least 1+ years of case management experience
Licensed Practical Nurse (LPN) with a minimum of 3+ years of clinical experience with older adults
1+ years of experience with local behavioral health providers and community support organizations addressing SDoH
1+ years of experience with MS Office, including Word, Excel and Outlook
1+ years of experience in long term support services or working with older adults
Reside in the state of Indiana
Ability to travel 75% of the time within assigned territory
Access to reliable transportation & valid US driver's license
Preferred Qualifications
Demonstrated experience/additional training or certifications in Motivational Interviewing, Stages of Change, Trauma-Informed Care, Person-Centered Care
Experience working in team-based care
Experience in serving individuals with co-occurring disorders
Background in Managed Care
Bilingual in Spanish or other language specific to market populations
