Job Description
Join Monte Nido & Affiliates as a Social Worker and play a vital role in providing care coordination and case management for clients and families in our treatment programs. Key Responsibilities
Partner with the clinical team for case management and care coordination
Assist clients and families in connecting with community-based programs
Send referrals to community programs and facilitate collaboration
Help facilitate transitions between various levels of care
Provide necessary information and emotional support to clients and families
Ensure clients continue to receive care after transitioning
Required Qualifications
Master’s degree in Social Work or related field
Experience with CareOregon/Medicaid
Proven experience in case management or care coordination, preferably in behavioral health or residential treatment settings
Strong communication skills
Ability to work collaboratively with a multidisciplinary team
Ability to build relationships with clients, families, and community providers
Strong understanding of available community resources, government programs, and insurance processes
Preferred Qualifications
LCSW preferred
