Job Description

Key Responsibilities
Function as a case planner and develop action plans for client care
Conduct routine community contacts and home visits
Evaluate clients' living conditions and needs
Identify and refer clients to appropriate treatment and service providers
Provide case management and individual/group counseling
Perform community outreach
Required Qualifications
BA Degree or equivalent experience
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
