Key Responsibilities
Evaluate and establish a plan of care for patients referred for home health services
Facilitate admission of referrals upon discharge from the hospital to a home care program
Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners
Required Qualifications
Licensed Registered Professional Nurse in NYS
Minimum of two years experience in home care nursing
Proficiency in physical assessment and interviewing skills
