Key Responsibilities
Manage and provide the full scope of advanced nursing practice for targeted patient populations
Serve as the clinical authority on individualized care teams
Manage comprehensive advanced nursing care including assessments and interventions
Collaborate with patients, families, and primary care physicians
Ensure compliance with Patient-Centered Medical Home standards
Participate in on-call coverage schedule
Educate clients and caregivers on chronic disease management
Perform procedures as outlined in collaborative practice agreement
Participate in interdisciplinary team meetings and case conferences
Required Qualifications
License and current registration to practice as a Registered Professional Nurse in New York State
Certificate (license) and current registration to practice as a Nurse Practitioner in New York State
Master's Degree of Science in Nursing or other graduate degree from a nurse practitioner educational program
Current ANCC or AANP certification as an adult, family or geriatric nurse practitioner
Preferred Qualifications
Maintains credentialed status with VNS Health Medical Care at Home
Maintains NPI, Medicaid and Medicare provider numbers
Maintains a collaborative practice agreement with a physician
PhD in psychiatric-mental health Nurse Practitioner
