Job Description
Join The Jewish Board as a Case Associate, where you'll make a significant impact on the lives of clients by developing action plans and providing essential support services. This role involves community outreach, case management, and collaboration with social workers to ensure effective client care.
Key Responsibilities
Function as a case planner and supply appropriate interventions for client care
Develop action plans to effectively identify and utilize existing resources
Perform routine community contacts on behalf of social worker
Evaluate actual living conditions of clients through home visits
Provide structure to clients and evaluate independent living skills
Run groups under the supervision of a social worker
Perform individual/group counseling based on client needs
Conduct community outreach and establish linkages with supportive organizations
Provide case management and maintain necessary documentation
Required Qualifications
BA Degree with some experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
