Assess patients, develop individualized treatment plans, and provide therapy to improve speech, language, cognition, and swallowing function. Working closely with physical and occupational therapists, you’ll ensure comprehensive care while educating patients and caregivers on effective communication and swallowing strategies. You’ll also maintain accurate documentation to uphold SNF regulations and deliver high-quality, patient-centered care.
