Key Responsibilities
Evaluate and treat patients according to standards of care
Coordinate patient services including specialty referrals and home health care
Participate in care team activities and daily huddles
Complete medical record documentation timely
Assist in setting a cooperative tone in practice
Monitor quality of care and outcome data
Engage in growth opportunities for services
Participate in local primary care on-call program
Ensure compliance with licensing and accrediting bodies
Focus on direct patient care
Required Qualifications
Current and unrestricted medical license in Texas
Graduate of accredited MD or DO program
Board Certification or Eligible in Family Medicine, Internal Medicine or Geriatric Medicine
Active and unrestricted DEA license
Excellent verbal and written communication skills
High level of skill with interpersonal relationships
Engaged in integrated team-based care model
Willingness to learn/adapt to value-based care setting
Superior patient/customer service skills
Basic computer skills including email and EMR
Compliance with TB screening program
Preferred Qualifications
Medicare Provider Number
2-5 years of directly applicable experience
Experience managing Medicare Advantage panel
Knowledge of Medicare guidelines and coverage
Knowledge of HEDIS quality indicators
