Title: Manager of Case Management Location: Riverside CA 92501 Shift: Full-Time – Admin Hours - Rotating Weekends (Monday - Friday 8 AM - 5 PM - this is an exempt position so hours may vary.) Key Requirements: Case Management experience and leadership experience is a must. It is important that the candidate will be able to start the role without training and can hit the ground running. Given the circumstances specific to Riverside they need to have Manager Candidates that have leadership experience and acute hospital case management experience. Acute care background, understands the role of CM with DC planning proactively. Understands insurance coverage and post-acute care criteria, able to have difficult conversations with families and patients regarding expectation around discharging. Able to critically think through scenarios that require a CM expertise with discharges. Minimum of 3-5 years of CM experience, must have prior leadership experience. Current CA Licensed Registered Nurse (will accept pending licensure). Additional Information Provided Aside from Job Description: Manager Roles & Duties: Focus on throughput and ensuring patients are moved along the continuum timely. This will take on various different tasks from helping the CM staff follow up on outstanding tasks to digging into records to see where gaps could be closed in progressions of care. Staff education and accountability on the process of care coordination, discharge planning, and company case management processes. Escalations of delays that are preventing the patients care from progressing, with continued follow up until addressed. Identification of barriers to patient throughput and partnering with facility teams to resolve barriers Additional Information: Manager position will report to Director of CM Direct reports will include RN Case Managers & Case Management Assistants 15-18 FTEs per Manager One Administrative Assistant in Department Total Number of Beds: 542 All Inpatient Units are serviced by Case Management Department Culture/Highlights: ICARE values that align with the Mission statement of the company. RCH is looking for Case Manager leaders that have a proactive approach to DC planning, and discussions with patients to ensure that there is no delay with the patients discharge. Shift & On-Call Details: Monday - Friday 8 AM - 5 PM - this is an exempt position so hours may vary. On Call is typically on a week-by-week basis divided between the CM Leadership Team. Calls are normally on addressing afterhours and troubleshooting issues that require service recovery or immediate need such as emergent HLOC transfers or transportation delay/issues. Weekend Call consists of being a resource to the team and facilitating approvals for LOA, HLOC, service recovery or transportation needs. #TB_HC
