Required Qualifications
Graduate of an accredited school of Certified Occupational Therapist Assistant
Washington Occupational Therapy Assistant License upon hire
National Provider BLS - American Heart Association upon hire
Medicare National Provider Identification Number (NPI) within 30 days of hire
2 years specialized occupational therapy training or experience
