Unit Notes:Department: LTC | RN*Certifications Required: BLS*- Type of Facility: 100 Residents Bed; 96 Semi-private beds, 4 private bed. 2 floors (1 LTC and 1 LTC-SNF). No locked units - Dementia and Alzheimer's patients wear a tracker that locks the door if they get close to it.- EMR: PCC in conjunction with paper charting.- Ratios: 1:17-20. If in the House Supervisor role then will be asked to oversee the whole facility. - Common diagnosis: LTC/SNF patient population: DM, COPD, Post Stroke, heart disease, fractures, dementia (Secure care tracker), Alzheimers, Rehab, chronic conditions, multiple comorbidities, and more.- Job Duties: Med passes, vitals, blood sugar checks and management, wound care, ostomy management, foley management, tube feeding administration, and assisting other team members.- Experience: Must have prior LTC/SNF experience. Minimum of 1 year of experience, prefer 2 years of experience. Will accept first time travelers, prefer experienced travelers.- Are there IV's or titratable drips?: Occasional IV antibiotics; potential PICC line management.- Are there Tele monitors?: None. - Are RN's required to attend team conferences for acute inpatient rehab?: They are required to attend Daily Huddle.- DON/Charge Nurse/House Supervisor: Supervisor on every shift, available as a resource.- Physician availability after hours: NP rounds each morning M-F, 2 MDs- 1 comes in on Tu & Fr evenings, 1 comes twice a month on Weds; Off- Hours: can call the hospital for on-call provider.- Interdisciplinary Team: PT, OT, SLP, Case Managers, SW, and Dieticians.- Is RT on-site 24/7: No RT in house, LPNs & RNs expected to manage respiratory treatments and equipment: Cpap, NC, High; no trachs or vents.- Pharmacy on-site? Medication dispensing system?: Bubble Packs for patient prescription medications; House stock of generic meds (tylenol, etc).- Shifts & Scheduling: Days: 7 A-3 P or 7-7 (typically 3 RNs and/or LPNs, 4-5 LNAs on each floor & 1 MNA), Evenings: 3 P-11 P, Nights: 11 P-7 A, 7-7 (typically 2 RNs and/or LPNs, 4-5 LNAs on each floor). Scheduler makes out the schedule 6 weeks in advance; travelers asked to stay flexible and help to fill gaps in permanent staffing. DON & ADON in house 7a - 4p.- Weekend Requirements: Every other weekend. - Holiday Requirements: May be asked to work holidays.- Call or Standby Requirement: None; Covered by perm staff.- Floating Requirements: Can float between either floor.- Dress Code: Any color scrubs, must look professional, no big earrings/jewelry, no fake nails; hair must be up; no face tattoos.- Department Specific Orientation: 3 shifts with a preceptor.*Radius Rule: 50 miles and must note in submission if clinician has ever worked for Coos County.*Entered on: 4/23/2024 Cancellation Policy:2 week notice Charting System:Point Click Care & Paper BSN Required:False License to Submit:Yes Trauma Level: Care Setting:Post Acute Division:Nursing Shift Notes: Required Certifications:BLS
