Job Description
Join Danbury Brunswick as a Memory Care Coordinator, where you'll play a crucial role in enhancing the lives of residents with dementia through tailored therapeutic programs. Be part of a supportive work family that values both employee and resident well-being.
Key Responsibilities
Plan and coordinate therapeutic programs for residents
Assess resident characteristics and organize program content
Monitor daily functioning of the neighborhood
Assist in developing and conducting staff training on memory care programs
Establish a sense of teamwork through communication and meetings
Market the program through community involvement
Maintain accurate documentation in compliance with regulations
Assist with resident admissions and transitions
Required Qualifications
Must be an LPN
Two years of previous experience in programming including plan of care programs
Experience in coordinating meetings with POAs and families
Experience working with dementia residents in an assisted living environment
Preferred Qualifications
3-5 years of experience in memory care
Background in nursing providing one-on-one care for seniors
Flexible schedule including availability for evenings, weekends, and holidays
