Job Description

Key Responsibilities

  • Plan and coordinate therapeutic programs for residents
  • Assess resident characteristics and organize program content
  • Monitor daily functioning of the neighborhood
  • Assist in developing and conducting staff training on memory care programs
  • Establish a sense of teamwork through communication and meetings
  • Market the program through community involvement
  • Maintain accurate documentation in compliance with regulations
  • Assist with resident admissions and transitions

Required Qualifications

  • Must be an LPN
  • Two years of previous experience in programming including plan of care programs
  • Experience in coordinating meetings with POAs and families
  • Experience working with dementia residents in an assisted living environment

Preferred Qualifications

  • 3-5 years of experience in memory care
  • Background in nursing providing one-on-one care for seniors
  • Flexible schedule including availability for evenings, weekends, and holidays
