Key Responsibilities

Required Qualifications
Bachelor's degree in social work, psychology, nursing, or related fields
Two years of experience in direct services or case management for mentally disabled, chronically ill, or homeless individuals
Experience in interdisciplinary teams
Experience in care management or care coordination in medical or behavioral health environments
Preferred Qualifications
Fluency in a second language such as Spanish, Russian, or Creole
