Key Responsibilities
Perform licensed practical nursing functions and services as directed by the Registered Nurse.
Complete assessments on residents and make appropriate interventions.
Administer medications according to facility policy.
Participate in the care planning process and update plans of care as needed.
Comply with standards of professional documentation.
Communicate and manage the delivery of resident care.
Guide staff in meeting the needs of residents.
Participate in physician rounds and communicate changes in resident condition.
Seek educational opportunities and assist with orientation of new staff.
Investigate incidents and facilitate conflict management.
Required Qualifications
Licensed Practical Nurse (LPN) - active license in the state of practice
Preferred Qualifications
Less than 1 year experience in Long Term Care
