As a Field Family Nurse Practitioner, you will provide essential screening and preventive primary care services in the comfort of patients' homes and communities. This role involves comprehensive medical assessments, care coordination, and collaboration with healthcare teams to enhance patient outcomes, particularly for underserved populations.
Key Responsibilities
Provide general medical care and care coordination to various patient levels
Perform comprehensive evaluations including history and physical exams
Address both chronic and acute primary care complaints
Establish and document reasonable medical diagnoses
Order and perform diagnostic laboratory and radiology testing
Create and implement a medical plan of care
Schedule patient appointments
Provide post-discharge coordination to reduce hospital readmission rates
Perform face-to-face visits in various settings
Participate in community-based 'Pop Up Clinics'
Drive up to 120 miles a day to various locations
Collaborate with fellow nurse practitioners
Prescribe medications and perform procedures
Perform timely documentation in medical records
Required Qualifications
Master's degree in family health from accredited nursing program
Active and unrestricted national certification from an accredited organization
Current state-issued license to practice as a Family Nurse Practitioner
Current Basic Life Support for Healthcare Professional certification
Current unrestricted driver’s license
Advanced computer skills, proficient with Word, Excel, and Electronic Medical Record
Preferred Qualifications
3-5 years experience as a Registered Nurse and/or Nurse Practitioner
Experience in home health, community health, or public health setting
Previous experience in management of chronic conditions
