Job Description

Join UnitedHealthcare as a Field Care Coordinator, where you'll serve as a primary care manager for members with medical and behavioral needs. Engage with members, develop care plans, and collaborate with care teams to enhance health outcomes while making a significant impact on community health.

Key Responsibilities

  • Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment
  • Develop and implement person-centered care plans addressing medical, behavioral, and socioeconomic needs
  • Partner and collaborate with internal care team, providers, and community resources
  • Provide education and coaching for member self-management
  • Support proactive discharge planning and manage Care Transitions
  • Advocate for members and families to ensure needs and choices are represented

Required Qualifications

  • Bachelor’s degree in a health or human services field or LMHP, RN/LPN, QMHP, LMSW, LBSW, MSW or BSW
  • 1+ years of care coordination or behavioral health experience and/or work in a healthcare environment
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook
  • Experience working with members who have medical needs, the elderly, individuals with physical disabilities and/or those who may have communication barriers
  • Access to reliable transportation and the ability to travel within assigned territory to meet with members and providers

Preferred Qualifications

  • CCM certification
  • Experience working with Medicaid/Medicare population
  • Long term care/geriatric experience
  • Experience working in team-based care
  • Background in Managed Care
