Job Description

Key Responsibilities

  • Perform initial comprehensive assessments on newly enrolled patients
  • Develop and update patient management care plans
  • Meet with patients and legal representatives to review care plans
  • Provide basic nursing care procedures
  • Prescribe diagnostics and interventions to avoid unnecessary admissions
  • Complete follow-up and post-discharge assessments
  • Consult with clinical teams following patient transfer
  • Educate patients on disease processes and wellness
  • Participate in meetings related to patient care and clinical excellence
  • Implement quality initiatives to improve care management

Required Qualifications

  • Completion of Nurse Practitioner or Physician Assistant accredited program
  • Current state RN/NP or PA license
  • Certification through AANP, ANCC, or NCCPA

Preferred Qualifications

  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of long-term care and community resources
  • Knowledge of CPT, ICD-10, and HCPC codes
  • Knowledge of clinical standards of care
  • Awareness of NCQA requirements and CMS guidelines
