Job Description

Key Responsibilities
Perform initial comprehensive assessments on newly enrolled patients
Develop and update patient management care plans
Meet with patients and legal representatives to review care plans
Provide basic nursing care procedures
Prescribe diagnostics and interventions to avoid unnecessary admissions
Complete follow-up and post-discharge assessments
Consult with clinical teams following patient transfer
Educate patients on disease processes and wellness
Participate in meetings related to patient care and clinical excellence
Implement quality initiatives to improve care management
Required Qualifications
Completion of Nurse Practitioner or Physician Assistant accredited program
Current state RN/NP or PA license
Certification through AANP, ANCC, or NCCPA
Preferred Qualifications
2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
Knowledge of long-term care and community resources
Knowledge of CPT, ICD-10, and HCPC codes
Knowledge of clinical standards of care
Awareness of NCQA requirements and CMS guidelines
