Job Description

Key Responsibilities
Engage members face-to-face and/or telephonically for comprehensive needs assessments
Develop and implement person-centered care plans
Collaborate with internal care teams and community resources
Provide education and coaching for member self-management
Support proactive discharge planning and care transition management
Advocate for members' needs and choices
Required Qualifications
2+ years of case management experience serving members with Serious Mental Illness (SMI)
1+ years of case management experience serving elderly and/or persons with physical or developmental disabilities
1+ years of experience with MS Office, including Word, Excel, and Outlook
Driver’s License and access to reliable transportation
Preferred Qualifications
Bachelor’s degree in Psychology, Special Education, or Counseling
CCM certification
Experience working in team-based care
Experience in Managed Care
Social work experience
Bilingual
