Job Description:

The Physical Therapist (PT) is responsible for evaluating and treating patients in a skilled nursing facility to help improve mobility, restore function, relieve pain, and prevent disability. The PT will develop individualized treatment plans based on patients’ physical needs and work closely with interdisciplinary teams to ensure the best outcomes.

Key Responsibilities:

• Perform initial and ongoing physical therapy evaluations based on patients’ conditions and medical histories
• Develop and implement personalized care plans with defined, measurable goals
• Administer therapeutic exercises, manual therapy, modalities, and other interventions
• Educate patients and their families about exercises and techniques to aid recovery
• Monitor and document patient progress and adjust treatments as necessary
• Collaborate with physicians, nurses, and other healthcare team members
• Ensure documentation is timely and complies with federal and state regulations

Required Qualifications:

• Active and valid Physical Therapist license in the state of Massachusetts
• Graduate from an accredited Physical Therapy program
• CPR/BLS Certification (if required by the facility)
• Excellent interpersonal and communication skills
• Ability to work both independently and as part of a team
