Key Responsibilities
Conduct comprehensive bio-psychosocial assessments and establish care plans
Collaborate with interdisciplinary teams for care coordination
Utilize therapeutic methods in accordance with care plans
Monitor intervention progress and revise plans as needed
Provide education to patients and families
Maintain patient-centered documentation
Participate in administrative and development activities
Provide supervision to social work students
Support hospital programming development
Contribute to improved patient experience
Required Qualifications
Master's Degree in Social Work, Counseling, or Marriage and Family Therapy
LCSW, LADC
Previous experience in a healthcare setting
Valid Driver's License
Computer proficient
