Key Responsibilities
Manage the delivery of high-quality home care services
Ensure adequate staffing and appropriate patient assignments
Maintain the integration of interdisciplinary services
Facilitate achievement of agency goals related to clinical outcomes, patient experience, fiscal responsibility, and staff retention
Required Qualifications
Graduate from an accredited School of Nursing
Current NYS RN license and registration
Minimum of 3 years’ experience in a Certified Home Health Agency
Preferred Qualifications
BSN preferred
Supervisory experience
Knowledge of OASIS E and clinical outcome measures
Familiarity with Federal Conditions of Participation and NYS DOH regulations related to Home Health Care
