As a Speech Language Pathologist at CenterWell Home Health, you'll provide personalized therapeutic services to patients in their homes, focusing on rehabilitation and improvement of speech, language, and swallowing disorders. You will collaborate with healthcare teams to enhance patient quality of life and independence, making a meaningful impact on their recovery journey.
Key Responsibilities
Evaluate, direct and provide speech/language pathology service to patients in the home or facility
Participate in the development and periodic review of the Plan of Treatment and Plan of Care
Assess and treat disorders of speech, voice, language, hearing and swallowing
Administer and interpret diagnostic tests and therapeutic treatments
Observe, record and report changes in the patient's condition
Provide instruction and training in the use of alternative communication systems
Counsel and instruct patients, families, and healthcare staff
Maintain and submit required documentation
Participate in care coordination activities and discharge planning
Attend staff development programs and obtain continuing education
Required Qualifications
Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
Minimum of six months experience as a speech therapist / speech language pathologist
Current and unrestricted license
Current CPR certification
Good organizational and communication skills
Valid driver’s license, auto insurance, and reliable transportation
