- Minimum 5 years of acute inpatient case management experience (telephonic, utilization management, or therapy experience will not be considered)
- Acute discharge planning experience
- Recent (within the last year) inpatient case management experience
- Strong computer skills
- Proficiency in Epic EMR
- Ability to quickly adapt and work efficiently with minimal training
- Experience managing a typical case ratio of 1:21–25 patients (meetings with each patient based on individual risk factors)
Registered Nurse (RN) with a valid, unrestricted State of Michigan license OR
Licensed Master Social Worker (LMSW) with a valid, unrestricted State of Michigan license
- Basic Life Support (BLS) certification through the American Heart Association (AHA)
- Nursing Degree OR Master’s in Social Work (MSW)
