We are seeking a dedicated and compassionate Registered Nurse to join our rehabilitation hospital team. The ideal candidate will have a strong background in patient care, with a focus on rehabilitation, recovery, and functional improvement. This role requires excellent clinical skills, a collaborative approach, and a commitment to helping patients regain independence after illness, surgery, or injury.

Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized rehabilitation care plans.
• Administer medications and treatments according to physician orders, following all safety protocols.
• Monitor and document patient progress, vital signs, and functional gains accurately in the EMR system.
• Educate patients and families on recovery goals, home exercise programs, and ongoing care needs.
• Collaborate closely with Physical Therapists, Occupational Therapists, Speech-Language Pathologists, and physicians to coordinate patient care.
• Support patients through mobility training, ADL participation, and therapeutic activities.
• Ensure compliance with HIPAA regulations and maintain confidentiality of patient records.
• Participate in interdisciplinary team meetings, quality improvement initiatives, and discharge planning.

Qualifications & Experience

• Active RN license in PA or Compact license.
• Experience in one or more of the following settings preferred:

– Inpatient rehabilitation hospital or acute rehab unit
– Acute care, med-surg, or post-acute care
– Neurological, orthopedic, or stroke rehabilitation programs

• Familiarity with EMR/EHR systems such as Epic, Cerner, or Meditech.
• Strong knowledge of rehabilitation nursing principles, mobility support, and patient education.
• Excellent communication, critical thinking, and teamwork skills.
