Key Responsibilities
Interpret orders and transcribe or verify them accurately
Accurately compound and dispense pharmaceuticals
Monitor drug therapy regimens for interactions and appropriateness
Provide accurate medication information to healthcare staff and patients
Interact proactively with medical staff regarding drug therapy issues
Document clinical activities and interventions
Sustain the hospital formulary and minimize non-formulary procurements
Supervise and assist pharmacy technicians and support staff
Train and participate in Disaster Team drills
Lead compliance with medication turn-around time policy
Participate in staff meetings and development for pharmaceutical care
Support training and mentorship of new staff members
Serve as a preceptor for pharmacy students/residents
Support quality improvement and patient safety programs
Maintain professional competency and controlled substance records
Comply with regulatory standards and make medication recommendations
Participate in management of medical emergencies
Required Qualifications
B.S. or Pharm D. from an ACPE-accredited School of Pharmacy
Clinical pharmacy experience in a hospital setting
Active Pharmacist license in good standing
BLS & ALS/ACLS certification
Preferred Qualifications
Completion of an accredited PGY-1 Pharmacy residency or fellowship
2+ years clinical pharmacy experience without residency or fellowship
Doctor of Pharmacy from an ACPE-accredited college of pharmacy
