Job Description
Join The Jewish Board as a Case Associate, where you will play a vital role in supporting clients to lead independent lives. As a case planner, you will develop action plans, evaluate client needs, and connect them with essential community resources.
Key Responsibilities
Develop action plans for client care
Evaluate living conditions through home visits
Identify and utilize community resources for clients
Provide case management and individual/group counseling
Conduct community outreach
Maintain thorough documentation including notes and assessments
Required Qualifications
BA Degree
Experience in a community or social service group
Five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
