Key Responsibilities
Conduct complex psycho-social transition planning assessment and reassessment
Assist with adoptions, abuse and neglect cases
Care coordination
Lead multi-disciplinary patient care conferences
Communicate with patients and families about the plan of care
Collaborate with physicians and ancillary departments
Ensure timely documentation in Case Management system
Facilitate quality improvement initiatives
Required Qualifications
Master's of Social Work
Currently licensed or license eligible to practice as a LICSW, LCSW, or LMHC
Preferred Qualifications
2 years of acute hospital experience
