? Job Responsibilities

• Perform 60-minute evaluations and treatments
• Create, implement, and adjust individualized care plans
• Ensure safe, functional return to community activities
• Document thoroughly (evaluations, goals, charges, medical necessity, etc.)
• Educate patients, families, staff, and students
• Collaborate within interdisciplinary team and delegate tasks as appropriate
• Maintain compliance with safety, licensure, and documentation standards

? Required Skills & Qualifications

• Degree in Physical Therapy from an accredited program – Required
• Active PT license in the state of practice – Required
• 1+ year of outpatient experience – Required

• Current CPR certification – Required
