Job Description
Join The Jewish Board as a Senior Case Manager, where you will support adults with serious mental illness in their journey towards independence. Utilizing a compassionate approach, you will advocate for residents, help them create personalized recovery goals, and provide essential support to enhance their quality of life.
Key Responsibilities
Carry a caseload and practice person-centered and collaborative service planning with residents
Support residents in meeting individual recovery goals and independent living skills
Advocate on behalf of residents with other service providers
Complete regular and timely documentation of all resident interactions
Participate in intake, assessments, discharge, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
