Join Caretenders Hospice as an RN Case Manager, where you will make a meaningful impact on patients' end-of-life care through compassionate nursing and collaboration with families and healthcare teams.
Key Responsibilities
Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient
Conduct initial nursing evaluation to determine eligibility for hospice services
Identify and reassess patient's/family's needs regularly
Initiate and coordinate the plan of care while maintaining accurate clinical notes
Document problems, goals, interventions, and patient/family responses
Collaborate with the patient/family, attending physician, and other team members
Required Qualifications
Current RN licensure in the state of practice
One year of clinical experience
Current CPR Certification
Current driver's license and valid vehicle insurance
Preferred Qualifications
Experience in hospice or palliative care
