Job Description:
We are seeking a compassionate and skilled Physical Therapist (PT) to join a skilled nursing and long-term care facility in Pittsfield, MA, on a travel contract. The PT will evaluate residents, develop individualized treatment plans, and deliver therapy services to enhance mobility, independence, and quality of life. The ideal candidate is patient-centered, organized, and experienced in geriatric rehabilitation.
Key Responsibilities:
Perform patient evaluations to assess mobility, strength, and rehabilitation needs.
Develop and implement individualized treatment plans in collaboration with the care team.
Provide therapeutic interventions including strengthening, gait training, balance activities, and mobility programs.
Educate residents, families, and caregivers on therapy goals, safety practices, and home exercise programs.
Document evaluations, progress notes, and outcomes accurately and timely.
Collaborate with interdisciplinary healthcare staff to ensure comprehensive and effective care.
Promote resident safety, independence, and quality of life through evidence-based therapy.
Qualifications:
Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.
Current and valid Physical Therapist license in Massachusetts (or compact license).
Minimum of 1 year of PT experience, preferably in long-term care or skilled nursing facilities.
Current Basic Life Support (BLS) certification.
COVID vaccine and recent TB test.
Strong communication, organizational, and clinical assessment skills.
Compassionate approach with experience working with geriatric patients.
