Join the Youth Assertive Community Treatment (ACT) team as a Licensed Behavioral Health Clinician, where you will provide essential treatment and support services to youth and families facing significant behavioral health challenges. This role emphasizes individualized care and collaboration with families and community resources to ensure effective transitions back to the community.
Key Responsibilities
Provide treatment to children and their families addressing clinical needs
Engage families as active partners in care development and modification
Assess risk, develop safety plans, and provide crisis intervention services
Participate in the development of person-centered care plans
Monitor discharge and ongoing care planning
Establish relationships with community providers and act as a liaison
Provide on-call after hour crisis intervention services
Required Qualifications
Licensure (LMSW, LCSW, LMFT, LCAT) in a Behavioral Health or related field
Minimum of one year of experience in providing direct services to children with serious emotional disturbance and their families
