Key Responsibilities
Deliver skilled nursing services in-home including wound care, medication administration, diabetes care, post-surgical care, injections
Work 1:1 with patients, building trust and rapport
Collaborate closely with the care team and the patient's physician
Keep clear, accurate documentation and comply with DOH standards
Required Qualifications
Licensed Practical Nurse (LPN) licensed in Pennsylvania
CPR-certified
Able to travel locally with own vehicle
Preferred Qualifications
Experience in home health or long-term care
