Job Description
Join the Youth Assertive Community Treatment (ACT) team as a Licensed Behavioral Health Clinician, where you'll provide critical support and treatment to youth and families facing significant behavioral health challenges. This role involves working collaboratively within a multi-disciplinary team to develop personalized care plans and enhance family functioning, all while making a meaningful impact in the community.
Key Responsibilities
Provide treatment to children and their families to address clinical needs.
Engage youth and families in care planning and goal-setting.
Assess risk and provide crisis intervention services.
Participate in developing and implementing person-centered care plans.
Monitor discharge and ongoing care planning.
Establish collaborative relationships with community providers.
Complete casework documentation and report data as required.
Provide on-call after-hour crisis intervention services.
Required Qualifications
Licensure (LMSW, LCSW, LMFT, LCAT) in a Behavioral Health or related field
Minimum of one year of experience providing direct services to children with serious emotional disturbance and their families
