Job Description

Join The Jewish Board as a Case Manager, where you'll support adults with serious mental illness in achieving their recovery goals. You'll work closely with residents to foster independence, navigate complex care needs, and advocate for their well-being in a compassionate environment.

Key Responsibilities

  • Carry a caseload and practice person-centered service planning
  • Complete regulatory service plans with residents
  • Advocate for residents with service providers
  • Facilitate support groups and meetings
  • Document resident interactions and progress
  • Link residents to community-based treatment

Required Qualifications

  • Bachelor of Arts (BA) or Bachelor of Science (BS) degree
  • Experience working with individuals living with serious mental illness, substance use disorders, or trauma

Preferred Qualifications

  • At least 1-year experience in Human Services
