Key Responsibilities
Provide evidence-based diabetes education to patients and families
Participate as a member of interdisciplinary healthcare team
Consult and resource for clinical staff on diabetes education
Evaluate and improve diabetes education programs
Ensure correct documentation in patient's medical record
Conduct group presentations and educational programs
Required Qualifications
Associates Degree in Nursing
Minimum of two (2) years of clinical nursing experience
Minimum two (2) years diabetes education experience
Current California Nursing license
Certification of Diabetes Care and Education Specialist (CDCES) required within 6 months of hire
