Job Description

Join UnitedHealthcare as a Field Care Coordinator, where you will play a critical role in managing care for individuals with long-term service needs. You will implement person-centered plans, conduct home visits, and coordinate with healthcare teams to deliver efficient and effective care. This position allows for significant flexibility with a field-based and home-office arrangement.

Key Responsibilities

  • Develop and implement care plan interventions
  • Communicate health-related information to stakeholders
  • Advocate for patients' needs and choices
  • Assess and implement individualized care strategies
  • Identify barriers to care and provide interventions
  • Initiate referrals for social services
  • Provide resource support to members
  • Manage person-centered service/support plans
  • Conduct home visits and in-person visits
  • Document and maintain member information compliance

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan or a Master's/Bachelor's degree in Social Work with appropriate licensure
  • 2+ years of experience in community health settings
  • 1+ years of experience with local behavioral health providers
  • 1+ years of experience with long-term care needs
  • 1+ years of experience with electronic documentation systems
  • Access to reliable transportation
  • Ability to secure Protected Health Information (PHI) in designated workspace
  • Ability to travel to Southfield, MI for quarterly meetings
  • Must reside within the state of Michigan

Preferred Qualifications

  • RN or LMSW; LCSW, LLMSW
  • 1+ years of medical case management experience
  • Experience in Motivational Interviewing, Stages of Change, Trauma-Informed Care
  • Experience serving individuals with co-occurring disorders
  • Experience with MI Health Link (MMP)
  • Experience in Managed Care
  • Knowledge of NCQA documentation standards
