Key Responsibilities
Gather patient information from various sources
Maintain master patient index and audit trail
Retrieve medical records for authorized professionals
Document and communicate actions and irregularities
Work with state officials during surveys
Maintain the facility's clinical record system
Perform audits for compliance including patient charts and medical records
Required Qualifications
High school diploma or equivalent
Completion of a certificate or 2-year associate's degree in MT or HIM, or 3-5 years equivalent experience in a skilled nursing or hospital setting
Two years or more experience working with medical records
Preferred Qualifications
Licensed RHIT, CPC or CCS with coding experience
Licensed as Registered Health Information Technician
