Join U.S. Renal Care as a Dialysis Social Worker where you'll provide critical social services to patients and their families, promoting their psychological adjustment to dialysis. Key Responsibilities
Assist with clinical and operational processes to improve patient health
Coordinate transient arrangements
Identify and counsel psychosocial issues
Provide patient and family education
Act as advocate on patient's behalf with local, state, and federal agencies
Participate in interdisciplinary team activities for patient assessment and care planning
Maintain collaborative relationships with medical staff and community resources
Required Qualifications
Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE)
Current licensure in applicable state (unless in AZ, PA, or Guam)
Basic computer skills including Microsoft Office
Demonstrated working knowledge of the English language

Previous experience in providing social services to dialysis patients
