Job Description
As a Case Manager, you will play a critical role in transforming healthcare for our members by developing and facilitating personalized care plans, ensuring high-quality outcomes for mental and behavioral health needs. This remote position involves local travel for member visits and collaboration with community resources.
Key Responsibilities
Evaluate member needs via phone or in-home visits
Develop ongoing care plans for high acuity members
Coordinate care between members, families, and providers
Monitor care plans and member status for changes
Provide education to members and families on treatment options
Required Qualifications
Master's degree in Behavioral Health or Social Work
2 – 4 years of related experience
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN with BH experience
Preferred Qualifications
Pediatric experience
Bilingual Spanish speaking
