Position Summary

We are seeking two experienced Registered Nurse Care Managers for 13-week assignments at our main campus in York, Maine. The RN Care Manager will coordinate concurrent utilization management activities across the hospital and serve as the central liaison between patients, healthcare teams, the organization, and payors throughout the hospital stay.

Key Responsibilities

• Conduct comprehensive assessments of patient needs to identify and secure resources necessary for successful transitions of care.
• Lead and collaborate with an interdisciplinary team through daily care rounds, care conferences, and care planning activities.
• Communicate effectively in verbal, written, and electronic formats, including hospital software and email systems.
• Coordinate with community organizations to arrange for durable medical equipment, insurance authorizations, and patient transportation.
• Monitor and participate in managing inpatient length of stay to support efficient, high-quality care delivery.
• Apply knowledge of hospital reimbursement methods, payor requirements, and quality measures relevant to care management and utilization review.

Education, Licensure & Qualifications

• Required:
  • Active Registered Nurse (RN) license.
  • Associate’s or Bachelor’s degree in Nursing.
  • 5 years of recent clinical experience in medical/surgical nursing, home care, hospital case management, and/or utilization review .
  • Experience in discharge planning and utilization review required.
  • Active BLS certification.
• Preferred:
  • CCM (Certified Case Manager) or ACM (Accredited Case Manager) certification.
  • Certification from a nationally recognized case management or utilization review organization.
