Key Responsibilities
Provide on-call patient visits for urgent physical and psycho-social issues
Coordinate with Call Center Triage and On Call Manager
Respond to Triage visit requests via cell phone
Implement and educate on changes to the plan of care
Attend patient deaths to provide emotional support and guidance
Report to triage and primary Hospice team regarding patient visits
Required Qualifications
Graduate from an accredited School of Nursing
New York State RN licensure
Preferred Qualifications
At least two years experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing
NYS valid drivers license
National board certification CHPN within one year of hire
