Key Responsibilities
Serve as primary care manager for high medical risks/needs members
Engage members to complete comprehensive needs assessments
Develop and implement individualized care plans
Collaborate with internal care teams and community partners
Provide referrals and linkages as appropriate
Required Qualifications
Current, unrestricted independent licensure as a Registered Nurse in Indiana
Master’s degree in social work
Bachelor’s degree in social work, psychology, special education, or counseling with 1+ year of case management experience
Licensed Practical Nurse (LPN) with 3+ years of clinical experience with older adults
1+ years of experience with local behavioral health providers and community support organizations
1+ years of experience with MS Office
1+ years of experience in long-term support services
Resident of Indiana
Ability to travel 75% of the time
Access to reliable transportation & valid US driver's license
Preferred Qualifications
Experience in Motivational Interviewing, Stages of Change, Trauma-Informed Care
Experience working in team-based care
Experience serving individuals with co-occurring disorders
Background in Managed Care
Bilingual in Spanish or other market-specific languages
