Key Responsibilities
Complete clinical assessments of members and determine eligibility for case management
Develop and implement case management plans in collaboration with members and healthcare professionals
Conduct telephonic and face-to-face visits as required
Monitor care plans for effectiveness and document interventions
Maintain ongoing member case load for outreach and management
Promote integration of behavioral health care and long-term services
Facilitate interdisciplinary care team meetings
Assess for barriers to care and provide coordination assistance
Required Qualifications
Bachelor's or Master's Degree in social science, psychology, gerontology, public health, or social work
1-3 years in case management, disease management, managed care, or medical/behavioral health settings
Active, unrestricted license if required
Valid driver’s license with good driving record
Preferred Qualifications
3-5 years in case management, disease management, managed care, or medical/behavioral health settings
Licensed Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), Certified Case Manager (CCM), Certified in Health Education and Promotion (CHEP), Licensed Professional Counselor (LPC/LPCC), Respiratory Therapist, or Licensed Marriage and Family Therapist (LMFT)
