Key Responsibilities
Provide on-call patient visits for urgent physical and psycho-social issues
Coordinate with Call Center Triage and On Call Manager
Respond to Triage visit requests via cell phone
Communicate effectively with the on-call team and Hospice physician
Implement and educate on plan of care, including medication management
Attend patient deaths to provide emotional support and guidance to family
Report patient visit details to triage and primary Hospice team
Required Qualifications
Graduate from an accredited School of Nursing
New York State RN licensure
At least two years experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing
Preferred Qualifications
NYS valid driver's license
Word, Excel, Windows applications
Certification in Hospice and Palliative Nursing (CHPN) within one year of hire
