Job Description
Join The Jewish Board as a Senior Case Manager, where you will support adults with serious mental illness in achieving independence and improving their quality of life. You'll provide essential advocacy and guidance, ensuring residents meet their personalized recovery goals while fostering a respectful and compassionate environment.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Advocates on behalf of residents with service providers
Completes documentation of resident interactions and progress
Participates in intake, assessments, discharge, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
