Key Responsibilities
Provide services to help progress patients through the continuum of care
Screen for post-acute needs and transition planning
Coordinate care and make community care referrals
Monitor plan effectiveness and reassess need for Care Management services
Required Qualifications
Nursing degree/diploma required upon hire
Bachelor's degree in Nursing or higher within 3 years of hire
Oregon Registered Nurse License upon hire
2 years Acute care hospital or community based experience
1 year Experience with electronic medical records or documentation systems
1 year Home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience
Preferred Qualifications
Bachelor's degree in nursing upon hire
CCM certification upon hire
