Job Description:

We are seeking an experienced Travel RN Case Manager to join the team in Santa Rosa, CA. The ideal candidate will have a strong background in acute hospital case management, care coordination, and discharge planning in a fast-paced environment.

Key Responsibilities:
  • Care coordination and discharge planning
  • Patient needs assessments and DME orders
  • Coordination with interdisciplinary teams and external partners (HMOs, SNFs, etc.)
  • Documentation in Epic (required)
  • Participate in family meetings and treatment plan development
  • Must independently arrange SNF placement and ambulance transport
Requirements:
  • Minimum 2 years of acute hospital Case Management experience
  • Prior travel RN experience required
  • Experience using Epic (required)
  • BLS certification
  • Must commit to 3 of 5 major holidays (Thanksgiving, Christmas Eve/Day, New Year's Eve/Day)
  • Must be willing to float within the state if required
  • 2 supervisory references within the last 3 years
  • Must complete skills checklist prior to submission
  • Must have recent experience (within last 2 years) in Case Management
