Job Description
As a Licensed Practical Nurse at Sunshine State Health Plan, you'll play a crucial role in transforming healthcare for our members. You will develop and manage personalized care plans, focusing on mental and behavioral health needs, while collaborating with providers to ensure quality care and support.
Key Responsibilities
Develop and facilitate complex care management activities
Conduct evaluations via phone or in-home visits
Develop ongoing care plans for high acuity members
Coordinate care between members and providers
Monitor care plans and member outcomes
Educate members and families on treatment options
Required Qualifications
Master's degree in Behavioral Health or Social Work
Degree from an Accredited School of Nursing
2 - 4 years of related experience
Licensed Master's Behavioral Health Professional (LCSW, LMSW, LMFT, LMHC, LPC) or RN
Preferred Qualifications
Bilingual candidates
Experience in pediatric care
