Job Description

Join Cooper University Health Care as a Navigator Specialist, where you will provide critical care coordination and case management for individuals with Substance Use Disorder. You will play a vital role in linking patients to essential services, fostering supportive relationships, and ensuring continuity of care across various settings.

Key Responsibilities

  • Provide ongoing care-coordination and case management services
  • Guide linkage to appropriate clinical services including medication-assisted treatment, behavioral health treatment, and social services
  • Foster supportive relationships with individuals with Substance Use Disorder
  • Provide continuity of care by meeting patients in various clinical settings
  • Identify health and wellness needs and barriers for patients
  • Perform community outreach and act as patient advocates
  • Attend clinical and programmatic conferences and meetings

Required Qualifications

  • Bachelor's degree in a related field with 1 year of relevant experience working with patients with Substance Use Disorder
  • Associates degree with 2 years of relevant experience
  • Certified Peer Recovery Specialist designation with 2 years of relevant experience
  • Certified Medical Assistant with 4 years of relevant experience
  • Valid driver's license
  • Own vehicle or reliable alternate means of transportation

Preferred Qualifications

  • Experience in case management or care coordination
  • Knowledge of behavioral health treatments
