Key Responsibilities
Provide ongoing care-coordination and case management services
Guide linkage to appropriate clinical services including medication-assisted treatment, behavioral health treatment, and social services
Foster supportive relationships with individuals with Substance Use Disorder
Provide continuity of care by meeting patients in various clinical settings
Identify health and wellness needs and barriers for patients
Perform community outreach and act as patient advocates
Attend clinical and programmatic conferences and meetings
Required Qualifications
Bachelor's degree in a related field with 1 year of relevant experience working with patients with Substance Use Disorder
Associates degree with 2 years of relevant experience
Certified Peer Recovery Specialist designation with 2 years of relevant experience
Certified Medical Assistant with 4 years of relevant experience
Valid driver's license
Own vehicle or reliable alternate means of transportation
Preferred Qualifications
Experience in case management or care coordination
Knowledge of behavioral health treatments
